Job Title: Clinical Leader
Department: Long Term Care
Location: DeMay Living Center
Hours Per Week: 40
Schedule: Days, 7a-330p
Sign-On Bonus: Up to $20,000
SUMMARY:
The Clinical Leader is a Nursing Leader, who serves as a resource for problem solving, provides clinical expertise, facilitates staff mentoring and coaching, and facilitates resident flow.
RESPONSIBILITIES:
Round on staff and physicians.
Using unit census, round on all residents daily, using key words at key times to identify issues
Ask residents if there is staff that should be recognized
Resolve any issue or concerns at the time of the rounding
Follow up any concerns with Nurse Manager
Complete rounding log and turn in to Nurse Manager
Identify and have knowledge of all high risk residents (including residents in restraints, residents with previous falls or at high risk for falls, residents with skin breakdown, residents with a long LOS, and admissions and transfers to the unit), ensure plan of care is current
Recognize staff publicly, for a job well done or for role modeling
Write thank you notes to staff
Coach staff privately when performance is below standard
Initiate counseling and/or assist with disciplinary action process
Make assignment based on needs of residents and competency of staff
Ensure all LPN staff are assigned to an RN
Ensure appropriate number of staff available on unit at all times; assign and monitor lunch, break, and committee meeting coverage
Assist staff with care as needed
Communicate any staffing overages or needs to Nurse Manager and/or Off Shift Director
Maintain staffing with in budgeted hours per resident day for census
Evaluate staffing for next shift, day, week
Consistently plan on unexpected admissions and facilitate admissions
Address issues delaying resident discharges
Communicate with Clinical Bed Coordination regarding all admissions, transfers, and discharges if appropriate
Assist staff in care to expedite flow and bed availability
Facilitate admissions by accepting admission and transitioning to assigned nurse
Coordinate discharges and admissions for the day
Ensure compliance with Resident Safety Goals
Ensure emergency equipment is checked daily
Ensure refrigerator logs are completed daily
Ensure compliance with Infection Control policies
REQUIRED QUALIFICATIONS:
Current New York State Registered Nurse License
Effective 1/1/2014, BLS required for all new candidates prior to hire
BLS required for all internal team members by 12/31/2014
PREFERRED QUALIFICATIONS:
Associates degree in Nursing required; Bachelor’s of Nursing preferred.
2 years RN experience preferred
For Long Term Care: experience with frail, elderly/geriatric population preferred
EDUCATION:
- AS: Nursing (Required)
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $42.18 - $55.43
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
