Job Title: Intake Coordinator Healthy Moms
Department: Healthy Moms
Location: St. Mary's
Hours Per Week: 40
Schedule: Monday to Friday 8:30am to 5pm

SUMMARY:

Responsible for facilitating intake processes, including the review of referral information, conducting prescreen interviews, scheduling clinical consultations, and facilitating intake appointments with treatment providers.

RESPONSIBILITIES:

  • Review and process referral information, determining completeness, appropriateness, and need for additional evaluations.

  • Coordinate and schedule intake screenings, interviews, and consultations, ensuring smooth completion of the intake process.

  • Communicate effectively with clients, clinical, and administrative staff to support referral and intake decisions.

  • Manage appointment schedules, data reporting, and logistical needs such as patient transportation.

REQUIRED QUALIFICATIONS:

  • High School diploma or equivalent

  • 1 year of work experience in a clinical, office or healthcare setting

PREFERRED QUALIFICATIONS:

  • Excellent oral and written communication skills for routine interaction with program staff, patients, community members

  • High attention to detail & highly organized

  • Exhibits excellent critical thinking/analysis skills

  • Ability to work independently and as part of a team

  • Experience with electronic medical record system (preferably Epic) & Microsoft Excel

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

PAY RANGE: $19.00 - $25.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
