Job Title: Speech Pathologist
STATUS: Per diem
LOCATION: 49 Lawrence Ave, Potsdam, NY 13676
DEPARTMENT: Speech Pathology
SCHEDULE: Days, 7:00 a.m. - 5:00 p.m. Weekends every 3rd week on call; shift duration 8 hours
ATTRIBUTES
- Minimum of nine (9) months of supervised work experience (completion of CFY), or participation in supervised clinical fellowship, for a minimum of 36 weeks under a NYS licensed Speech Pathologist, followed by the NYS licensure examination and achievement of NYS license
- One (1) year of experience with the frail elderly population preferred
- Compassionate, warm, and patient focused
- Excellent organization and planning skills
RESPONSIBILITIES
- Evaluation & Planning. Perform a complete initial evaluation of the referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family, and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence
- Documentation. Maintain a complete documentation of patient evaluations, treatment, progress and discharge status; send required information to appropriate professionals
- Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals and assisting in discharge planning as appropriate
EDUCATION:
- MS: Speech Pathology (Required)
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $32.80 - $47.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
