SUMMARY
The Home Care Coordinator is responsible to facilitate the patient discharge from the facility setting to certified home care services in the
patient’s residence. The Home Care Coordinator will provide superior customer service by modeling the Brand Promise and Core Values.

SCHEDULE: M-F, 8:00am-4:30pm
ATTRIBUTES
- Experience in working in partnership with the interdisciplinary healthcare team.
- Excellent interpersonal skills (strong written and verbal communication skills).
- Experience utilizing an Electronic Medical Record and Microsoft office
- Demonstrate ability to apply decisive, sound, objective clinical judgment for cases with varying complexity.
RESPONSIBILITIES
- Evaluates eligibility, needs and care resources for each patient referred to RRH Home Care. Obtains insurance information and authorizations for appropriate reimbursement.
- Completes a comprehensive clinical assessment for home care services and develops a home care plan with the patient/caregivers to ensure a safe home care plan.
- Obtains MD order to support home care plan.
- Coordinates all in- home services including but not limited to professional, paraprofessional services, durable medical supplies, equipment, etc, as the care plan dictates.
- Completes all required documentation in patient’s facility record and RRH Home Care required documentation per requirements.
- Gathers data, analyzes data and completes all assigned reports.
- Collaborates with facility care coordinators, social workers, and physicians to find solutions for challenging patients requiring home care as a part of the facility discharge plan.
- Participates in ongoing RRH Home Care, Home and Community Based Service, and/or UHS committees or groups.
- Represents RRH Home Care at appropriate Community Groups
- Act as a CHHA resource and represent RRH Home Care in the community in various health care settings (ie hospital, physician office, etc.).
- Participates in marketing efforts as assigned. Assumes role community resource expert in providing education as assigned.
MINIMUM QUALIFICATIONS
- AAS in Nursing; BSN preferred.
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $73,528.00 - $98,072.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
