The Physical Therapy Assistant (PTA) provides and documents physician ordered occupational therapy services in a home care setting. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values.
STATUS: Per Diem
LOCATION: Park Ridge Living Center
DEPARTMENT: Long Term Care
SCHEDULE: Flexible Schedules
Associates Degree from an accredited Physical Therapy Assistant program.
HHCS Only: Minimum of 2 years post-graduate experience as a PTA in an acute care setting and/or an acute rehabilitation facility.
Rehab Only: Successful completion of all fieldwork requirements and eligible to sit for PTA or OTA exam.
Documentation: Completes and documents (within agency standard) related to patient care including visits notes, renewal orders and verbal orders. Includes progress or lack of goal achievement in all charting.
Treatment: Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Provides PT gait and ambulation training. Instructs in the use of assistive devices. Provides instruction in safe transfer using appropriate body mechanics and equipment.
Team: Confers with therapist directly, physician and/or interdisciplinary team members regarding patient response to therapy and any concerns. Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census, patient needs.
