Responsibilities
The Nurse Evaluator is responsible for evaluating new NHTD Waiver participants and current participants following a significant medical event that may have altered the individual’s cognitive or physical abilities. Evaluate and make recommendations regarding level of care changes for waiver participants. Provide superior customer service by modeling the Brand Promise and Core Values.
STATUS: Full Time
LOCATION: 89 Genesee St. 5th floor
DEPARTMENT: NYS DOH Grant
SCHEDULE: Monday-Friday; 9AM-5PM
This position will be required to travel throughout the Western NY region. The counties include Monroe, Genesee, Wayne, Yates, Livingston, Seneca and Ontario. Mileage reimbursement is available.
ATTRIBUTES
Two years satisfactory professional experience in working with individuals with disabilities and/or seniors in a community setting.
NHTD/TBI Waiver experience preferred
UAS Training Preferred
RESPONSIBILITIES
Evaluate and make recommendations regarding level of care changes for waiver participants.
Meet with prospective waiver participants to complete an intake interview and determine if the individual is a probable candidate for this waiver.
Make referrals to the appropriate LDSS to determine Medicaid eligibility; if appropriate, offer applicant the opportunity to participate in the NHTD waiver.
Make referrals to other available services if the individual is found not appropriate for participation in the NHTD waiver program.
Work in coordination with the LDSS to appropriately use Medicaid State Plan and NHTD waiver services for eligible applicants.
Interview potential service providers and make recommendations to DOH based on factors identified in provider manual.
Train service providers and offer technical assistance on the needs of the waiver participants, development and implementation of comprehensive waiver Service Plans and standard documentation and reporting requirements Review, monitor, and approve NHTD waiver participant Service Plans developed by the participant and the Service Coordinator.
Once approved, the Plan shall be updated and submitted to the RRDS every six months to assure the quality and cost effectiveness of all provided waiver services.
Participates in educational programs to increase professional competence and to meet personal needs and goals.
Attends all mandatory in-services.
