SUMMARY

As a Registered Nurse Care Manager at Jefferson Family Medicine, you will work in collaboration with providers and care teams to identify and proactively manage the needs of patients with high risk or complex medical, behavioral health and/or psycho-social problems. You will work to improve health outcomes and break down barriers to success through practice, community and home based visits and telephonic support. As an RN in this role, your knowledge and network of community resources will be essential to our patients!

STATUS: Part time (20 hours per week)

LOCATION: 924 Jefferson Ave, Rochester, NY 14611**

DEPARTMENT: Care Management

SCHEDULE: Monday-Friday, 4 hours per day.

**Position requires regular travel between Jefferson Family Medicine and Riedman Campus.

ATTRIBUTES

  • A minimum of 5 years of experience in a care management/community health, utilization management or related role is required.
  • BSN within 5 years of hire into the position is required.
  • Excellent communication and interpersonal skills.
  • Must be adaptable and able to accommodate change on a regular basis.
  • Ability to use and navigate a computer and Electronic Medical Record (EMR) system.

RESPONSIBILITIES

  • Care Plans. Develops and implements a care management plan based on patient goals, preferences and disease states to promote improved health care outcomes and quality of life. 
  • Community Resource.  Links patients to appropriate community resources, facilitates referrals to appropriate care services, supports patient self-management, and communicates with providers in order to reduce barriers to improved health care outcomes.
  • Team Player. Serves as an integral member of the primary care practice's care team, assesses patients for risk of adverse health outcomes, and measures the impact of care management interventions.
EDUCATION AS: Nursing (Required)
LICENSES/CERTIFICATIONS BLS - Basic Life Support - American Heart Association (AHA); RN - Registered Nurse - New York State Education Department (NYSED)

Clinical Ladder

Nurses who advance on our clinical ladder receive a promotion at the bedside through growth in their skillset, a new title and monetary recognition up to $3.00/hour

Rapid Pay Increases

Receive rapid pay increases every six months during the first two years of employment, and annual increases thereafter.

Unlimited Certifications

Nurses earn up to an additional $1.00/hour for each certification earned plus a $1,000 bonus for the first certification earned, and $500 for the second certification.
