SUMMARY
As a Care Facilitator, your analysis of and attention to the care plan are key contributors to ensuring patient needs are met and delivery of care is optimal.
St. Lawrence Health (SLH) was established in December 2013 with the mission to improve health and to expand access through coordination and integration of services. SLH became an affiliate of Rochester Regional Health (RRH) in January 2021. Working together with our community partners, including local health departments and agencies, we are focused on disease prevention, promoting access to quality healthcare services, and improving the overall quality of life for our communities.

ATTRIBUTES
Required: Associates degree from an accredited School of Nursing, Bachelor’s Degree preferred, and/or comparable clinical experience.
Required: Licensed in New York State as a Registered Nurse.

RESPONSIBILITIES
Evaluation & Care Management. Conduct clinical reviews utilizing objective criteria to assign patient to the appropriate level of care; identify clinical resources required to achieve optimal patient outcomes and appropriate reimbursement; monitor resource utilization, perform continued stay reviews and assist with managing the length of stay of patients; refer cases to the Review Physician Advisor to validate level of care; communicate clinical information to payors
Documentation. Document level of care and medical necessity in the Care Management database to support the clinical review process
Leadership & Communication. Work with Care Management Leadership to identify problems, recommend solutions and work toward resolution; utilize data, trends and reports to proactively identify opportunities to improve processes and practice; function as a resource to healthcare team for payor requirements, ensuring optimal use of patient healthcare benefits; foster relationships with payors and facilitate communication between hospital and payor as appropriate
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $70,000.00 - $90,500.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
