SUMMARY
As a Clinical Leader of Long-Term Care, you are a Nursing Leader who serves as a resource for problem solving, provides clinical expertise, facilitates staff mentoring and coaching, and facilitates patient flow. Your keen attention to detail, along with your demonstrated empathy for patients, makes you a valuable resource for the unit.
Unity Living Center, a 120-bed state-of-the-art skilled nursing facility, is located on two recently renovated floors on the Rochester Regional St. Mary's campus.
Unity Living Center focuses on providing treatment and rehabilitative care for patients with medically complex needs, dementia, and behavioral challenges. Special services include short-term rehabilitation, pain control, IV therapy, peritoneal dialysis, access to hemodialysis, ventilator beds and relaxation therapy to meet the needs of those with chronic diseases.

ATTRIBUTES
- One (1) year of nursing experience required, Post-Acute nursing experience preferred
- One (1) year of supervisory experience preferred
- Exceptional documentation, communication and interpersonal skills
RESPONSIBILITIES
- Unit Management. Oversee and assure short-and long-term Nursing Department staff schedules; serve as a clinical resource to staff in problem solving, assessment and special procedures; coordinate with the Nursing Staff resident units and admissions department in the rapid admission of residents
- Supervision Oversees the dining areas, interacting with residents, families and staff in providing a safe, pleasant mealtime and assure appropriate completion of required records
- Patient Care. Facilitate units in emergency medical assessment and care of residents, including interventions, transportation and other resources as needed
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
LICENSES/CERTIFICATIONS:
- BLS - Basic Life Support - American Heart Association (AHA)
- RN - Registered Nurse - New York State Education Department (NYSED)
PAY RANGE: $39.00 - $51.25
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
