OUR Department:
Endocrinology provides care to the adult population. We treat a variety of diagnoses such as: Diabetes, Thyroid Disorders, Osteoporosis, Hormone Imbalance, PCOS, Weight Management, and help support the Transgendered community.
SUMMARY
Provides clinical assessment and diabetes-specific education on acute care patients currently on insulin or screened through laboratory results.
STATUS: Full-time

SCHEDULE: M-F, Days
ATTRIBUTES
Associates or Bachelor's in Nursing
Two to three years of diabetes specific experience
Registered in NYS as a registered nurse professional.
Certified Diabetes Educator (CDE), or must be obtained within within 2 years of hire.
RESPONSIBILITIES
Assesses and provides feedback to providers on interpretation of insulin protocols and offers other treatment options.
Helps develop standards and processes to provide streamline patient diabetes education both in the hospital and at home, though will not generally provide this information themselves.
Acts as a resource for discharge planning, including recommendations on home diabetes treatment plan, referral to outpatient education and endocrinology care.
Primary focus is to improve diabetes patient and hospital outcomes.
Keeps medical staff updated on the most recent diabetes information through profession development.

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $62,000.00 - $85,000.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
