SUMMARY

The Outpatient Social Worker provides customized clinical interventions, that are evidence-based, to treat underlying, root causes of psychosocial and environmental symptoms. Serves as a liaison between medical professionals and patients to increase collaboration and positive medical outcomes.

STATUS: Full Time

LOCATION: Hybrid, RGH and Linden Oaks

DEPARTMENT: Social Work

SCHEDULE: Monday-Friday, 8am-4:30pm

ATTRIBUTES

  • Required Licensure: NYS LMSW or LCSW

RESPONSIBILITIES

  • Assessment. Provides Social Work assessments that identify the underlying root cause of psychosocial and environmental symptoms. Completes diagnostic screening tools such as the PHQ screenings, Edinburgh, CAGE, and GAD 7.

  • Emergency Assistance. Provides crisis intervention with an immediate safety action plan for patients with suicidal, homicidal, hallucinating, substance abuse, and interpersonal violence/abuse issues.

  • Treatment Plan Development. Creates a care plan and links patient with services to address the needs of the patient. Manages referrals regarding psychosocial needs and determines appropriate interventions and strategies to meet those needs. Creates customized interventions for specialty populations. Diagnoses patients using the DSM V.

  • Documentation. Documents all services in accordance with clinical guidelines and policies

EDUCATION:

  • MS: Social Work (Required)

PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

LICENSES/CERTIFICATIONS:

  • LMSW - Licensed Master Social Worker - New York State Education Department (NYSED)

PAY RANGE: $60,500.00 - $76,000.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
