Schedule: Monday - Friday Days

SUMMARY

Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values.

Edna Tina Wilson Living Center is a 120-bed skilled nursing facility located in North Greece, NY. The Edna Tina Wilson Living Center uses an innovative neighborhood design, with resident rooms clustered around the activity center and the living and dining areas, to promote social and interactive living.

RESPONSIBILITIES

  • Within standards of practice established for a PTA, completes and documents (within agency standard) related to patient care including visits notes, renewal orders and verbal orders. Includes progress or lack of goal achievement in all charting.

  • Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician and/or interdisciplinary team members regarding patient response to therapy and any concerns.

  • Provides PT gait and ambulation training. Instructs in the use of assistive devices.

  • Provides instruction in safe transfer using appropriate body mechanics and equipment.

  • Participates in discharge planning, provides timely and adequate patient and family teaching to facilitate discharge plan.

  • Communicates regularly with Physical Therapist who oversees patient’s plan of care.

  • Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census, patient needs.

  • Maintains professional growth through active participation in continuing education programs, departmental in-services, etc.

  • Communicates pertinent information to patient and family members including goals, procedures, progress, etc. as appropriate.

REQUIRED QUALIFICTIONS:

  • Associates Degree from an accredited Physical Therapy Assistant program.

  • Current NYS PT Assistant licensure required.

EDUCATION:

  • AS: Physical Therapy (Required)

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

LICENSES/CERTIFICATIONS:

  • PTA - Physical Therapist Assistant certification - New York State Education Department (NYSED)

PAY RANGE: $32.00 - $32.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
