Job Title: Licensed Practical Nurse

Department: 2700 - Telemetry Unit

Location: Unity Hospital

Hours Per Week: Full Time - 36 hours

Schedule: Nights, 7p-7a, every other weekend and holiday.

SUMMARY:

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. The Licensed Practical Nurse is a key member of the care delivery team. The Licensed Practical Nurse communicates the needs of the patient and the team; collaborates with the multidisciplinary team, takes direction from the RN and delegates appropriately to the other members of the team.

The Telemetry Unit – 2700

The Telemetry Unit is a 22 bed, short stay, telemetry med/surg unit that provides care to patients who have an illness in which a complete evaluation and/or treatment is expected to take longer than 8 hours but less than 48 hours, and who are expected to be discharged home or admitted to the Hospital based on the clinical course and response to treatments. Diagnoses on this unit are primarily cardiac in nature, but the unit also cares for patients with a diverse variety of general medical illnesses.

RESPONSIBILITIES:

  • Patient Care & Service - Promote and restore patients' health by completing the nursing process; collaborate with physicians and multidisciplinary team members; perform various treatment procedures; provide physical, educational and emotional support to patients, friends and families; supervise assigned team members.
  • Medication Administration & Reporting - Administer medication and give/receive relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
  • Documentation - Ensure concise, pertinent and complete documentation using computerized medical record process.
  • Preparation - Ensure clinical areas are properly supplied and exam rooms are available and stocked.

REQUIRED QUALIFICATIONS:

  • Graduate from an accredited School of Practical Nursing or equivalent required.
  • Current licensure in State of New York as a Licensed Practical Nurse.

PREFERRED QUALIFICATIONS:

  • Strong communication, customer service, and interpersonal skills highly desired.
  • At least 6 months experience preferred.

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

LICENSES/CERTIFICATIONS:

  • LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
  • BLS - Basic Life Support - American Heart Association (AHA)

PAY RANGE: $24.75 - $30.75

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
