SCHEDULE: Full Time, Day/Evening/Night - ability to make you own schedule with minimum 16 hours weekly

SUMMARY
As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.
Position Summary:
Provides assistance in participants daily routine, activities and personal care at their place of residence.
Key Responsibilities:
Helps to provide a comprehensive plan to meet clients needs, daily as well as long term.
Maintains records of client’s daily activities, including general condition of client.
Aids in client’s daily routine, activities include but are not limited to, personal care, light housekeeping, errands and meal preparations.
Accurately and timely completes aide activity sheets, submitting activity sheets on a timely basis as specified by department policies.
Attends a minimum of 12 hours of annual in-service education.
Communicates any concerns/changes in client condition to appropriate staff in a timely manner and documents on Aide Activity sheet.
Keeps family and staff informed of clients activities and concerns.
Completes annual skill competency program per NYS regulation
Performs other duties as required.
MINIMUM QUALIFICATIONS:
Ability to meet one of the following eligibility requirements for Competency:
HHA Certified (Per NYS DOH regulations, the Certified Home Health Aide must have provided compensated services within the previous 24 consecutive months.)
Currently certified CNA along with completion of written test and one day HHA Skills competency class within 4 days of hire.
Documented home health aide or nurse aide training and competency evaluation from an out-of-state training program
Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months-Fundamentals of Nursing and must show completion of written test and one day HHA Skills competency class within 4 days of hire.
Veteran who was trained in the United States Military as medical technician or medic must show completion of written test and one day HHA Skills Competency Class within 4 days of hire.
LICENSURE:
Valid NYS Driver’s License required
PHYSICAL REQUIREMENTS: Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. Valid licensure, physical readiness, and flexibility for extended or on‑call schedules are typically required.
LICENSES/CERTIFICATIONS:
- NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)
- HHA - Home Health Aide certification - NYS Office of Professions
PAY RANGE: $18.65 - $20.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
