LOCATION: Physical Therapy & Rehabilitation - Ridgeway

SCHEDULE: Day Shift, Monday - Friday, 8-hr. Shifts

POSITION SUMMARY
Upon referral, evaluates patients. Develops and implements rehabilitative treatment programs for the purpose of restoring or improving the patient’s optimal level of function, independence, and quality of life. Documents and communicates results and recommendations. Participates in multidisciplinary activities. Participates in continuing education programs within as well as outside the hospital. Provides supervision to students during their affiliation. Participates in quality improvement programs and projects.

RESPONSIBILITIES

  • Evaluation & Planning. Perform a complete initial evaluation of the referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family, and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence

  • Documentation. Maintain a complete documentation of patient evaluations, treatment, progress, and discharge status; send required information to appropriate professionals

  • Collaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals, and assisting in discharge planning as appropriate

REQUIRED QUALIFICATIONS

  • Master's degree in Speech-Language Pathology or Communication Disorders (may be MA, MS, Med)

  • Required Licensure/Certification Skills: New York State licensure

  • or
    For recent graduates: Completion of at least 9 months supervised Clinical Fellowship year OR
    Eligible to participate in a supervised Clinical Fellowship year by a NYS licensed Speech Language Pathologist with a NYS Limited permit, followed by the NYS licensure examination and achievement of the NYS license.

PREFERRED QUALIFICATIONS

  • 1 year of experience in a medical setting preferred.

EDUCATION:

  • MS: Speech Pathology (Required)

PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $50.00 - $50.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
