Description

Process and manage a minimum productivity expectation of 25-30 referrals per week while maintaining quality, accuracy, and timeliness.

  • Review and evaluate referrals using clinical judgment to determine appropriate skilled home care service needs.
  • Complete and document home care referrals accurately and efficiently within designated electronic systems.
  • Communicate professionally and effectively with physicians, providers, managers, supervisors, hospitals, community agencies, and referral sources regarding patient care needs and referral status.
  • Educate patients and families regarding home care services, including service expectations, admission processes, and available resources.
  • Respond to patient, family, provider, and referral source inquiries promptly, professionally, and compassionately.
  • Gather, review, and verify demographic, clinical, insurance, and payer information necessary for service eligibility and authorization.
  • Utilize working knowledge of Medicare, Medicaid, commercial insurance plans, and other payer requirements related to home care services.
  • Demonstrate excellent telephone etiquette and customer service during all interactions.
  • Collaborate with interdisciplinary team members to ensure continuity of care and successful patient admissions.
  • Maintain effective communication with team supervisors and leadership regarding referral volume, barriers, and workflow needs.
  • Ensure compliance with organizational policies, regulatory requirements, and patient confidentiality standards.

  • Excellent verbal and written communication skills.
  • Strong interpersonal skills with the ability to communicate effectively with patients, families, providers, managers, supervisors, and interdisciplinary teams.
  • Strong typing skills and computer literacy.
  • Proficiency with electronic medical records, referral management systems, Microsoft Office applications, and other healthcare software platforms.
  • Knowledge of healthcare insurance plans, authorization requirements, and reimbursement processes.
  • Ability to utilize clinical reasoning and critical-thinking skills to identify and assess skilled care needs.
  • Ability to prioritize competing demands and manage a high-volume workload while maintaining attention to detail.
  • Demonstrated ability to work independently while contributing positively within a team environment.
  • Strong organizational, problem-solving, and customer service skills.
  • Commitment to maintaining confidentiality and professionalism in all interactions.

PHYSICAL REQUIREMENTS: Sedentary - Sedentary roles are primarily office-based and require prolonged sitting (67–100% of the workday) with minimal physical effort and lifting limited to under 10 lbs. Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver’s license.

PAY RANGE: $25.00 - $31.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
